<p><strong>Background:</strong> Tibia shaft fractures are most common lower limb long bone fractures. In approximately 80% of patients, tibia fracture occurs in diaphysis region. The procedure intramedullary interlocking nailing is done by positioning the injured extremity at minimum 90 degree flexion knee joint to achieve optimal exposure correct entry point. tension quadriceps tendon causes a typical apex anterior angulation proximal fragment. Supra patellar with semi extended position, reducing angulation. aim this study outcomes supra access for fractures.</p><p><strong>Methods:</strong> It descriptive longitudinal which was carried out from October 2019 September 2021 Dr. Balasaheb Vikhe Patil rural medical college, Loni, Ahmednagar. 60 case closed were treated supra-patellar technique.</p><p><strong>Results:</strong> our we selected, patients admitting institute. All underwent operative form nail suprapatellar approach. Out all approach, excellent results good range movements functional scale score.</p><p><strong>Conclusions: </strong>Supra-patellar approach found provide outcome 1/3<sup>rd</sup> fracture.</p>
